End-of-Life can be an overwhelming experience for many patients and their families. Vantage Hospice is dedicated to providing care and services that address needs such as support, reassurance, counseling, education, and resources that will guide and assist the patient and family through this journey.

Respite is designed for patients who live at home with their family and provides up to 5 days of care at a contracted nursing facility. This program is available to assist with family members who are experiencing fatigue, stress, or just may want to take a small vacation and know that their loved one will be cared.

Continuous care is designed to assist with symptom control of a patient who has not been responsive to medication changes in the plan of care and documentation substantiates the need for crisis intervention. This program provides up to 24 hour of nursing care at the beside of patient to adjust medication as symptoms occur. Continuous care will end when patient becomes comfortable and symptoms are fully managed or the patient has passed away.

In-Patient care is provided when a patient is in crisis and symptoms are better managed at a facility wherein 24 hour nursing is available to stabilize and manage symptoms as they occur.

The basis of standardized hospice care; home visits by interdisciplinary team members to manage the overall well-being of patient and family which vary in frequency and length according to need.

On call services are an afterhours program provides nursing and psychosocial staff to ensure any issues or concerns can be addressed at all times, each are available to make visits when there is a change in condition or crisis.

Once a patient is admitted to hospice services, a dedicated nurse will visit within the first 24 hours to ensure medications and equipment has arrived, to address any issues or concerns that may have occurred since admission, and to provide additional support and education.

Medications are delivered to the home of the patient in order to ensure physical comfort is maintained and symptoms controlled.

We provide medical equipment including, but not limited to, a specialized medical bed, oxygen, wheelchair, walker, bedside table, and shower chair.

 

We provide supplies for hygiene such as diapers, chux, gloves, lotion, hair care and oral care supplies.

After the patient passes away, Vantage Hospice will remain available to the family for the next 13 months to provide continued support through the family’s grieving process.
